Leverage the EHR to advance high-quality, evidence-based care for older adults
Older adults account for 50% of inpatient admissions, experience longer lengths of stay, and face a disproportionately high risk of complications. At the same time, hospitals and health systems continue to face growing workforce strain, rising patient complexity, and increasing operational pressures. Together, these challenges underscore the urgent need for scalable, system-wide approaches that improve care delivery, outcomes, and operational reliability for older adults.
This six-month implementation-based learning collaborative is designed to help hospitals and health systems operationalize and standardize evidence-based care for older adults through EHR-enabled workflow redesign and scalable deployment strategies.
Drawing on the four interconnected focus areas from Mass General Brigham’s West Health Accelerator, the initiative integrates evidence-based practices, technology, and team-based workflows into practical packages that hospitals can adopt and scale. The goal is to create a health care blueprint for older adults that is adaptable to hospitals of all types and sizes nationwide, from small community hospitals to large academic medical centers.
This collaborative is intended for hospital and health system leaders empowered to drive implementation and improvement, such as quality, population health, and nursing or clinical leaders who are accountable for outcomes and play a critical role in leading change within their organizations.
Participants will engage in a structured, implementation-focused experience designed to support real-world operational deployment and measurable organizational change over six months.
Monthly Virtual Sessions:
Interactive sessions focused on operationalizing evidence-based older adult care through EHR-enabled workflow redesign, clinical standardization, workforce efficiency strategies, and scalable implementation approaches. Each session includes practical assignments to support real-world implementation within participating organizations.
Technical Office Hours:
Monthly EHR-focused office hours provide dedicated support and troubleshooting for your implementation efforts.
Virtual Peer Exchange Platform:
Connect with peer organizations to share strategies, discuss barriers, and exchange resources throughout the program.
Assessments and Progress Tracking:
Pre- and post-assessments will measure baseline knowledge, readiness, and progress. These tools help ensure learning objectives are met and identify areas where additional support may be needed.
